PROBLEM SOLVED! After stumbling upon a thread somewhere with TONS of people experiencing the same problem... turns out it was a problem with the mouse. I have a Razer DeathAdder mouse that has 2 side thumb buttons. At one point a long time ago, one of the thumb buttons stopped working correctly...
