Windows 7 Library not updating correctly

Most often when I've recently saved a file from Word or other program and then go to insert it from the library, it does not show up even after several refreshes. I'll go back to word and check to see if I've saved the file correctly and it says it's there. What goes here? Why aren't files showing up correctly in the Librarys.
