Windows 7 printing .pdf files

I downloaded a .pdf file (china visa application form) I suppose to print out. In print preview only a single line prints. Reader is installed as C:\Program files (x86)\Adobe\Reader 10.0\Reader\Acro Rd32.exe) Please advise. Thank you.
