Have you tried updating your driver?
Post make and model and we'll help you find an update

Is your gpu overclocked?
If it is or your using an app to 'tweak it' please return to default values or stop using any apps involved.

Have you checked your gpu temperature when it has the issue?
This app is pretty good as it gives you a a max and a minimum:

What is the make and model of your computer? Video card? Video driver version? Power Supply?
