Defective video card.
You can use Driver Sweeper to clear the ATI drivers/settings, in safe mode. Then boot to normal and install the driver:
Drivers & Support | GAME.AMD.COM
But it's unlikely to help. If not, replace the card.
Also update bios to latest before doing so.
