Windows 7 Dubugging shows BSOD "probably caused by Ntfs.sys, ntkrnlmp.exe, win32k.sys."

Hello all! I have been trying to fix a 64-bit Windows 7 Professional workstation in a domain environment. Basic system info listed below:

  • Operating System: Windows 7 Professional 64-bit (6.1, Build 7601)
    • Service Pack 1 (7601.win7sp1_gdr.120830-0333)
  • System Manufacturer: Dell Inc.
  • System Model: OptiPlex 580
  • BIOS: BIOS Date: 06/19/12 14:12:07 Ver: 0.07 A07
  • Processor: AMD Phenom(tm) II X2 B55 Processor (2 CPUs), ~3.0GHz
  • Display: ATI Radeon HD 4200
  • Memory: 4096MB RAM
  • Available OS Memory: 3836MB RAM
  • Page File: 1613MB used, 6058MB available
  • Network Cards: 3 Installed
    • [01]: Broadcom NetXtreme Gigabit Ethernet
    • [02]: Compact Wireless-G USB Adapter

This PC is two years old and essentially started to crash out of the box. I put up with it for about a year-and-a-half because the crashes were somewhat infrequent, maybe occuring every two to three weeks. About three months ago I had become fed up with the crashes and purchased a software support contract with Dell. Although I had performed multiple diagnostic tests on my own to search for hardware failure (never found any issues), they made me rerun those same tests to prove there were no hardware issues. After the diagnostics came back clean, they told me I would have to reformat the hard drive and reinstall windows (which had come installed OEM). The good folks at Dell assured me this would be the only fix, and while I did not really agree with them, I complied with their suggestion.

Well, despite following their instructions, the PC in question continues to experience the occasional BSOD after reformatting. So whatever Dell thought the cause/cure would be, reinstalling Windows was not the answer to the problem. I'm not sure if this is a driver issue, or something completely different. When I run bugchecks on the Dump Files, the results typically state that the crashes were probably caused by Ntfs.sys, ntkrnlmp.exe, and win32k.sys.

Last weekend I ran memtest86+ for about 16 hours, checking each individual memory stick in each slot for 8+ passes, but no errors were found. I ran Driver Verifier for several days straight (still running it now) but could not crash the PC while stressing the third party drivers. If I can't isolate a hardware or a driver issue, then what exactly could be causing this? I have just about had it with this PC, so any help would be greatly appreciated!

One more thing I forgot to add; I did receive some advice to update the video driver a few weeks ago. I used a program called Driver Fusion from Treexy to remove all the AMD components from the PC that I could. I then downloaded a more recent version of the ATI
Radeon HD 4200 display driver v 8.970.100.3000, released 07/2012 (my previous version was v 8.850.0.0, released 04/2011). However, the PC has crashed 3 or 4 times since. So although it was probably a good idea to update the video driver, this alone was not enough to fix the issue.
 
Just FYI - I specialize in consumer systems. I have experience with workstations, but have very little experience with domain environments.

That being said, here we go!.............

Only 124 updates since SP1. Most systems have 140/150 or more. Please visit Windows Updates and get ALL available updates (it may take several trips there).

You have a Compact Wireless-G USB Adapter
I do not recommend using wireless USB network devices. Especially in Win7/Win8 systems.
These wireless USB devices have many issues with Win7(and I suspect with Win8) - using Vista drivers with them is almost sure to cause a BSOD.
Should you want to keep using these devices, be sure to have Win7/Win8 drivers - DO NOT use Vista drivers!!!
An installable wireless PCI/PCIe card that's plugged into your motherboard is much more robust, reliable, and powerful.
As a side note, we've had less problems with these devices recently - so I suspect that the newer drivers (late 2012) may have solved some of the issues).

From looking at the WER section of MSINFO32, it looks to be that the BSOD's are fairly random. This is usually a sign of either a low-level driver issue or a hardware problem. As such we'll focus first on the 3rd party drivers - and then we'll suggest hardware diagnostics (if you've run them already there's no need to repeat them).

Networking (wired) and wireless networking drivers date from 2009. Please update them. If Dell doesn't have more current versions, please post back and we'll see about getting them elsewhere.

Please update these older drivers. Links are included to assist in looking up the source of the drivers. If unable to find an update, please remove (un-install) the program responsible for that driver. DO NOT manually delete/rename the driver as it may make the system unbootable! :

Hey, thanks for the reply! I am in the process of applying some updates as I type this, and will continue to do so until there aren't any more to grab.

About the USB Wi-Fi; yeah, I know PCI/PCIe cards are better. I just installed this a week or two ago for trying to fix this workstation home on weekends. Durring the week it is physically plugged into the network. Although the driver may not help things, I know it's not the problem since BSOD predate its install. FYI, I plan on removing the driver once this PC is fixed and there is no need to bring it home.

I will remove/update the drivers that you highlighted for me and report back once I have done so.

I will probably run some more tests on the hard drive as well. Last night, before I went to bed and before you had replied to this thread, I ran SFC /SCANNOW and CHKDSK C: /F /R.

I ran SFC /SCANNOW twice, but I am a little confused by the results. The first time I ran the command, I received this message:

Windows Resource Protection found corrupt files and successfully repaired them. Details are included in the CBS.Log
windir\Logs\CBS\CBS.log. For example C:\Windows\Logs\CBS\CBS.log

So I run it again expecting it to say I'm all clear, and I receive this message:

So I checked out the CBS.Log file. Here is an excerpt:

I updated the network drivers. Will be checking out the firmware on my hard drive to see if any updates are available, and will then start running some diagnostics on hard drive. Updated computer info attached. If there's anything else you see that you think needs addressed, let me know.

I ran SeaTools for DOS but could not find any errors. As for as updating the firmware for my HDD, I'm not really sure how to go about doing that. My model number is: WDC WD2500AAJS-75M0A0 and my current firmware version is 02.03E02, but I couldn't find that on model number on the Western Digital site.

There were some errors that CHKDSK corrected, so there was a problem with the NTFS file system.
If the hard drive test passed, then there's likely not a problem with the hard drive. I haven't seen any firmware updates for your drive on the web.

The file that it repaired was a .chm file. That's a help file that isn't needed for proper functioning (in this case I believe that it's associated with the Disk Management snap-in for the Microsoft Management Console)

I'd suggest that we wait and see if there are any further BSOD's. If there are, zip up and upload the memory dumps (they're in C:\Windows\Minidump) and post them with your next post.
