Windows 7 BSOD on ASUS P6T-DV2 - Mostly When Trying to Load GPU Drivers

Asus P6T-DV2
12G RAM
Windows 7X64

Have had this configuration for nearly three years and it has run fine. Then recently BSODs started. I had not knowingly made any changes. System either freezes or the monitors go black. Windows says hardware 124 error but am not convinced. I have done this so far without any ability to see effects:
1) Fresh reload of Win 7 x64 Ultimate
2) Three different video cards (2 new) all tried in different slots
3) New memory
4) New motherboard (same model)
5) Ran CPU tester and came out OK

I have tried to update video card drivers and when either the AMD or NVIDIA driver packages (depending on which card I have inserted) try to install I get a freeze and have to reboot. I've spent a fair amount of time on this and would appreciate any help I can get. The attached .ZIP file provides all the info requested.

Only 110 updates since SP1, most systems have 130 to 140. Please visit Windows Update and get ALL available updates.

Please run these free diagnostics: Hardware Diagnostics
Post back with the results of the tests in order to let us know if they were done successfully or not.
Thanks!

All of the memory dumps are STOP 0x124 - which are generally hardware errors. There are couple of software issues to attempt fixing (they can also be to blame), but IMO the most likely cause is hardware. Please take a look inside the case and list every component that you haven't replaced (cables, PSU, hard drive(s), CD/DVD drives, mouse, keyboard, etc)

Here's the suggested software fixes:

Remove PHYLock/Image for Windows. TeraByte Unlimited Knowledge Base
This is just a hunch - but worst case is that you'll have to reinstall it again later on.
This will help to rule it out as a possible cause.

Please update these older drivers. Links are included to assist in looking up the source of the drivers. If unable to find an update, please remove (un-install) the program responsible for that driver. DO NOT manually delete/rename the driver as it may make the system unbootable! :

phylock.sys - this driver hasn't been added to the DRT as of this run. Please search Google/Bing for the driver if additional information is needed.

  1. Ran Windows update and it only had a couple of recent ones to download and they were “optional” ones. The only one I did not install was “Bing Desktop”
  2. I uninstalled PHYLock/Image for Windows.
  3. I ran Memtest86+ v4.20 through three passes with no errors noted and all 12G RAM installed.
  4. I updated Intel storage driver to version 11.7.1.1013 dated 11/19/12. Intel seems to have made a change in their drivers because when I went to device manager and checked the driver details it came up with two – iaStorA.sys and iaStorF.sys. Both are the version/date combo above. Seems to work OK but it is still early. These came from the latest Intel Desktop/Workstation/Server Express Chipset SATA raid controller package that came from Intel’s website. iastor.sys is still in the windows32\drivers folder as are iastorA.sys, iastorF.sys, and iastorV.sys
  5. I also looked to see if I could find an updated Marvell 61xx Raid Controller driver and found none. The one installed is dated 2/9/10 and is version 1.2.0.8000. The ASUS website has as their latest one for this motherboard Ver 1.2.0.69. Rolling back to this one may be something to consider if other attempts don’t work.
  6. I tried to run the AMD Catalyst Install Manager to see if it might work after the changes so far. Like before, it froze at the “detection” stage and I had to reset and reboot. I rebooted in safe mode and got the regular Stop 0124 error message at startup. I then tried to run the installer in safe mode. It failed again but this time gave me an error (vs. freezing entirely) that said “Failed to load detection driver”.
  7. I have not run the WD HDD utility. My HDs are in the onboard RAID configuration and I think I would need to pull them and run as an attached drive on another computer. I can do that but it slows things down quite a bit and I wanted to check and see, given the symptoms, if that was needed. Pls advise your thoughts.
  8. To revisit where things are. I can boot into the normal WIN 7 environment and in device manager everything looks fine. Windows loads a display adapter driver “ATI Radeon HD 5450” and I am able to use both monitors and have Windows extend across both at 1280 X 1024. Things are stable in this state and I can do what I need to do for most functions. If I try and install the AMD driver and application package for the video card it freezes as I described above and I get the Stop 0124 error. If I install my GT 440 video card Windows 7 hangs at the logo and freezes. The NVIDIA driver install package freezes. I can reboot into safe mode with the GT 440 but only get one monitor. All problems are associated with the video card and (apparently) communications with it for higher level functions.
  9. Things I have not replaced are:
CPU
Cables
Hard Drives
CD/DVD Player
Connected Memory Card Reader
Floppy Drive
Memory (I did swap memory out entirely and the problem remained, I put the old memory back in) and it passed the MemTest passes.

Please advise next actions based on this new information.

Regards,
Brent

I had serious issues with v1 of the P6T while using RAID 5 (this was using the Win7 beta and RTM)
I gave up on RAID and then purchased v2 of the P6T and have had no problems since.

Do not use the Marvell controller. If using it, please switch the devices to the Intel controller.
I can't recall if it's possible, but if it is, please disable the Marvell controller in the BIOS

Physically remove the floppy drive and disable it's controller in the BIOS (there may be 3 different places that the floppy is listed in the BIOS).
Disconnect the memory card reader from the Mobo
Disconnect the CD/DVD drive(s) - removing both the power and the data cables (after running the hard drive diagnostics). If needed you can feel free to reconnect them temporarily while we're testing.

Please run hard drive diagnostics on ALL hard drives (internal and external): Hard Drive Diagnostic Procedure
Try the bootable Seagate Seatools - as it may be able to test the drives separately without having to do anything with the RAID setup.

Run some of the CPU stress tests from the last entry on this page: Hardware Diagnostics
As time allows, please run the rest of the diagnostics (you've already run MemTest, so it's OK to skip that)
The video tests and Prime95 are the one's that I'm most interested in (after the hard drive tests).

What is the make/model/wattage/age of your Power Supply?

What I'm trying to do here is to rule out all other components. IMO the problem seems to be pointing at your motherboard, so if we rule out everything else, then it's all that's left to blame.
